A Medalha Real, também conhecida como "Medalha da Rainha", é uma medalha de prata entregue anualmente pelo monarca do Reino Unido por recomendação da Real Sociedade de Londres, sendo duas às "mais importantes contribuições para o avanço do conhecimento da Natureza" (uma para cada uma das grandes divisões da ciência) e outra para "contribuições extraordinárias no campo das ciências aplicadas". Estes prémios são conferidos a contribuições realizadas dentro da Comunidade Britânica.

O prémio foi instituído por Jorge IV do Reino Unido e atribuído pela primeira vez em 1826. O formato do prémio foi alterado em 1837 para abranger igualmente a matemática (as outras duas "grandes divisões da ciência" eram consideradas a física e a biologia), mas apenas cada três anos; em 1965, o sistema foi novamente mudado para permitir a atribuição anual das três medalhas.

Vários laureados do Prémio Nobel receberam também Medalhas Reais, como Frederick Sanger, Max Perutz e Francis Crick, e alguns que não são britânicos, também receberam, como Abdus Salam e Chintamani Rao, entre outros.
